Population health management

By using the Diabetes Data Dashboard you can identify and understand the needs of specific cohorts of patients and stratify the risk based on the nine annual diabetes key care processes*, current disease progression and optimisation of treatment therapies.

 

Data in one convenient place

The Diabetes Data Dashboard enables you to analyse data at a practice level and pro-actively review how well your patients are managing their diabetes. Aggregated data can also be provided at a Primary Care Network (PCN) level.

The Dashboard provides you with a tool that can simplify data by taking large patient lists and breaking it down into a manageable size, with the output presented in an easy to read and graphical way.

Risk stratification

The Dashboard provides you with the mechanism to identify which patients are in need of a review, escalation or intervention. 

If you wish to check which of your patients have not had their 9 NICE* recommended annual care processes, the clear presentation of the data makes it easy to interpret and navigate and progress where appropriate.

 

Medication Review

A medicines optimisation module enables a review of the patients prescribed medication and can assist you in managing your patient’s to achieve appropriate polypharmacy.

 

QOF/QAIF Review

The tool allows you to understand the variation in your QOF/QAIF data, and helps to target your interventions more accurately. There is also potential to improve your QOF/QAIF payments.

For example, a report can be run on those patients who have not received any of their 9 care processes*, and they can then be pro-actively invited to a clinic appointment.

Supporting Multidisciplinary Teams (MDT) reviews

The Dashboard can support MDT reviews by specialist diabetes teams across your PCN by providing easy access to specific patient data.
